Tympanoplasty is a surgical procedure used to treat a hole or tear in the eardrum, also known as the tympanic membrane. This surgery can also be used to repair or replace the three small bones behind the eardrum.

The eardrum is a thin membrane located between the outer ear and the middle ear that vibrates when sound waves hit it. Repeated ear infections, surgery, or trauma may cause damage to the eardrum or middle ear bones that must be corrected with surgery. Damage to the eardrum or the bones of the middle ear can lead to hearing loss and increase the risk of ear infections

Types of eardrum repair procedures

Myringoplasty

If the hole or tear in the eardrum is small, your doctor may first try patching the hole with gel or a paper-like tissue. This procedure takes 15 to 30 minutes and can often be done in a doctor’s office using only local anesthesia.

Surgical tympanoplasty

Tympanoplasty is performed if the hole in the eardrum is large or if you have a chronic ear infection that cannot be treated with antibiotics. You will likely be in the hospital for this surgery and will be under general anesthesia. You will be unconscious during this procedure.

First, the surgeon will use a laser to remove any excess tissue or scar tissue buildup in the middle ear. Next, a small piece of tissue from your vein or muscle sheath will be taken and grafted into the eardrum to close the opening. The surgeon will either pass through the ear canal to repair the eardrum, or make a small incision behind the ear and access the eardrum this way.

This procedure usually takes two to three hours

Ossiculoplasty

Osteoplasty is performed if the three small bones of the middle ear have been damaged due to ear infections or trauma. This procedure is also performed under general anesthesia. Bone can be replaced either by using bone from a donor or by using prosthetic devices.

Complications resulting from tympanoplasty

  • Complications from eardrum repair surgery are rare but can include:
  • Injury to the facial nerve or the nerve that controls the sense of taste.
  • Damage to the bones of the middle ear, causing hearing loss.
  • Vertigo.
  • Incomplete healing of the hole in the eardrum.
  • Moderate or severe hearing loss
  • Cholesteatoma is an abnormal skin growth behind the eardrum.

Preparing for tympanoplasty

The doctor must be informed about any medications and supplements the patient is taking, and the doctor must also be informed in the event of an allergy, including those related to drugs, latex or anesthetics. Be sure to tell the doctor if you are feeling ill as the surgery may need to be postponed.

You will likely be asked to avoid eating and drinking after midnight the night before surgery. If you need to take medications, take them with a small sip of water. Your doctor or nurse will tell you when to arrive at the hospital on the day of your surgery.

After an eardrum repair procedure

Your doctor will fill your ear with cotton wool. It must remain in your ear for five to seven days after surgery. A bandage will usually be placed over your entire ear to protect it. People who undergo eardrum repair are usually discharged from the hospital immediately.

You may be given ear drops after surgery. You should remove the cotton ball and place the drops in your ear. Then replace the pack and do not put anything else in your ear.

Try to prevent water from entering your ear during the healing period, avoid swimming, and wear a shower cap to keep water out. Also, avoid blowing your nose and if you need to sneeze, do so with your mouth open so pressure doesn’t build up in your ears.

Avoid crowded places and people who may be sick. If you catch a cold after surgery, this can increase your risk of developing an ear infection

After the surgery, you may feel pain in your ear or it may feel as if your ear is filling with fluid. You may also hear popping, clicking, or other noises in your ear. These symptoms are usually mild and improve after a few days.
